Company Account Requirements

To use a company account for payment, please present Staff/Work ID (if applicable) and one of the following at your appointment. You can also upload the below documents on the next step.

• A completed Service Authorisation Form or,
• Referral Letter,
• Printed Email clearly stating your authorisation on the Company account to be charged.
• Confirmation of medical clearance for yourself and any dependants.

Please be aware that failure to provide this information at your clinic appointment may result in fees needing to be paid by yourself at the clinic. If you are unsure about the above requirements, please check with your HR department or line manager prior to your appointment.
